Filing 8 ORDER: The August 30, 2018 rule to show cause is discharged; Plaintiff has paid all his outstanding filing fees. The court directs the Clerk of Court to (1) file Plaintiffs complaint #1 ; (2) issue summonses for service of the complaint on Defendants Yoksoulian, Stubenvol, Wolfe, and Spears; (3) revise the docket to correct the spelling of Yoksoulian's name; (4) terminate Sheriff Dart as a defendant to this action; and (5) send Plaintiff four blank USM-285 (Marshals service) forms, a magistrate judge consent form, filing instructions, and a copy of this order. Plaintiff must complete and return a USM-285 form for service on each Defendant. Failure to return the USM-285 forms by November 9, 2018, may result in dismissal of this case. Plaintiff also must promptly submit a change-of-address notification if he is transferred to another facility or released. If Plaintiff fails to keep the court informed of his address, this action will be subject to dismissal for failure to comply with a court order and for failure to prosecute. The court appoints the U.S. Marshal to serve Defendants Yoksoulian, Stubenvol, Wolfe, and Spears. Plaintiff's motion for attorney representation #4 is denied without prejudice. If Plaintiff wants to proceed with this action, he also must (1) submit a written statement, under oath and accompanied by a copy of his trust fund ledgers showing all activity in his account from August 1, 2018 through the present, explaining why he did not disclose on his application the $9,000 judgment he received in Calvin v. Dart, No. 18 C 1174 (N.D. Ill.); and (2) pay all of his outstanding filing fees or, alternatively, explain in writing and under oath why he should be allowed to initiate new lawsuits in this court while his filing fees remain unpaid.
