Filing 9 ORDER: Plaintiff's renewed application for leave to proceed in forma pauperis #8 is granted. The court assesses an initial partial filing fee payment of $6.00. The trust fund officer at Plaintiff's place of confinement is directed to make monthly deductions from Plaintiff's trust fund account in accordance with this order. The Clerk of Court shall send a copy of this order and a copy of the court's 8/19/16 order in Case No. 16 C 7515 to the trust fund officer at the Stateville Correctional Center. The Clerk shall also: (1) file Plaintiff's complaint #1 ; (2) issue summons for its service on Defendant Randy Pfister; (3) forward the USM 285 form Plaintiff submitted 5 to the U.S. Marshal; and (4) mail to Plaintiff a Magistrate Judge Consent Form and filing instructions for this Court. The Marshal is appointed to serve Pfister. Plaintiffs motion for attorney representation #4 is denied without prejudice. The Defendants other than Pfister are dismissed without prejudice.
