Filing 11 ORDER: Plaintiff's application for leave to proceed informa pauperis #10 is granted. The initial filing fee is waived. The Court orders the trust fund officer at Plaintiff's place of incarceration to make monthly deductions from Plaintiff's trust fund account in accordance with this order. The Clerk of Court is directed to electronically send a copy of this order to the trust fund officer at the Cook County Jail. However, summonses shall not issue. Plaintiff's amended complaint #9 is dismissed without prejudice. If Plaintiff wishes to pursue this litigation, by 2018, he must submit a second amended complaint complying with the instruction contained in the Court's previous Order. If Plaintiff fails to comply with this order the Court will summarily dismiss this case. The Clerk is directed to send Plaintiff an amended civil rights complaint form with instructions, a blank USM-285 service form, a copy of this order, and another copy of the Court's previous Order #5 .
