ORDER: Plaintiff has filed a complaint #1 and an application for leave to proceed in forma pauperis ("IFP") #10 . Plaintiff's IFP application demonstrates that she cannot prepay the filing fee, and the application is thus granted. Additionally, the Court has conducted an initial review of Plaintiff's complaint #1 and concludes that Counts I and II may proceed against the Illinois State Tollway Highway Authority. However, Counts III and IV are dismissed, and Union Local 73 is dismissed as a Defendant. The Clerk of Court is directed to: (1) file Plaintiff's complaint; (2) issue summons for service of the complaint on Defendant; and (3) mail Plaintiff a copy of this order, as well as one blank USM-285 (Marshals service) form. The U.S. Marshal is appointed to serve Defendant but will not attempt service until Plaintiff returns a completed service form. Plaintiff must return the completed USM-285 form to the Clerk of Court by December 11, 2018. Failure to do so may result in the dismissal of this case for lack of prosecution. ORDER: For the reasons stated herein, Plaintiff's application for leave to proceed in forma pauperis #4 is denied. If Plaintiff wishes to proceed with this case, she must either (1) pay the $400.00 filing fee; or (2) file an accurately and properly completed application showing that she cannot prepay the filing fee. Failure to comply with this order by October 23, 2018, will result in the dismissal of this case. The Clerk is directed to send Plaintiff a copy of this order and a blank application to proceed in forma pauperis. The Court defers initial review of Plaintiff's complaint #1 until the filing fee issue is resolved. Plaintiff's motion for attorney representation #5 is denied without prejudice.
