ORDER:Since filing this civil rights action, Plaintiff Andre Ferguson discharged from Cook County Jail. To proceed with the instant case, Plaintiff must submit: (1) written notification of his new address and contact information, (2) a copy of his jail trust fund account at the time of his release or some documentation showing the amount of funds in his account at the time of his release, and (3) an updated in forma pauper is application showing his current employment and financial status. See Robbins v. Switzer, 104 F.3d 895, 897-98 (7th Cir. 1997) (if a litigant is released from incarceration while his suit is pending, the Prison Litigation Reform Act still applies, he is still responsible for payment of the filing fee, and the court must assess his ability to pay). The Clerk of Court shall forward an IFP application form, along with a copy of this order, to Plaintiff at his last known address and his home address if court records have one. Plaintiffs failure to comply by December 15, 2018, will result in summary dismissal of this case. Signed by the Honorable Elaine E. Bucklo on 10/30/2018. Mailed notice. (pk, )
