November 19, 2018 Opinion or Order Filing 7 ORDER: The Court denies Plaintiff's application to proceed in forma pauperis #3 without prejudice. Plaintiff is directed to either (1) submit signed and completed application that includes all relevant information regarding his income, as explained below; or (2) pay the full statutory filing fee of $400.00. Failure to comply with this order by December 14, 2018, will result in summary dismissal of this case. Plaintiff is further notified that, although he has been released from custody, he owes the filing fee for this case and will be expected to pay it, if his income permits, in installments commensurate with his income or through trust fund account deductions if he is incarcerated again. The Clerk of Court is directed to send a copy of this order and an application for leave to proceed in forma pauperis to Plaintiff. The Court defers a merits review of Plaintiff's complaint #1 until Plaintiff has resolved his fee status. Plaintiff's motion for attorney representation #4 is denied without prejudice to later renewal. If he wishes to do so, Plaintiff may make an in-person appointment with the U.S. District Court Hibbler Memorial Pro Se Assistance Program Self Help Attorney by selecting the "representing yourself" button on the Court's homepage (www.ilnd.uscourts.gov) and then following the instructions for making an appointment. Alternatively, Plaintiff may call (312) 435-5691. Plaintiff must promptly inform the Court of address changes, or this case may be dismissed for want of prosecution.
