ORDER: On November 16, 2018, the Court denied Plaintiff's application for leave to proceed in forma pauperis as incomplete and ordered Plaintiff, by December 7, 2018, either to pay the full statutory filing fee or provide a completed application to correct the deficiencies identified in that order #6 . In a submission dated November 19, 2018, Plaintiff notifies the Court of an address change #7 . Given Plaintiff's change of address, the Court, on its own motion, extends the time for Plaintiff to comply with the instructions in the previous order to December 31, 2018. Failure to comply by that date will result dismissal of this case. The Clerk of Court is directed to send Plaintiff, at his current institutional address, a copy of this order and a copy of the Court's order of November 16, 2018 #6 , along with an application for leave to proceed in forma pauperis. Because the case is in its infancy, Plaintiff need only resolve his fee status at this time, and appears competent to complete that task without an attorney, Plaintiff's motion for attorney representation #4 also is denied without prejudice to renewal when his fee status is resolved. ORDER: Plaintiff's application for leave to proceed in forma pauperis #3 is denied because it is incomplete. If Plaintiff wishes to pursue this litigation, by December 7, 2018, he must either (1) submit a completed in forma pauperis application that is certified by an appropriate jail or prison official or officials and includes certified copies of his trust fund account statements from the beginning date of his incarceration through the date of his application, from each facility in which he has been incarcerated; or (2) pre-pay the full $400.00 statutory filing fee. Failure to do one of the foregoing by that date will result in dismissal of this case. The Clerk is directed to send a copy of this order to Plaintiff, along with an in forma pauperis application form. The Court defers screening Plaintiff's complaint #1 pursuant to 28 U.S.C. 1915A and ruling on any open motions until the resolution of Plaintiffs fee status. Signed by the Honorable Virginia M.
