ORDER: Plaintiff's motion to proceed in forma pauperis (IFP) #3 is denied without prejudice. The Court directs Plaintiff to submit a renewed IFP application that is certified by a trust fund officer; he must also attach copies of facility trust fund ledgers showing his income for the six months preceding the initiation of this action (that is, from April 23, 2018, through October 23, 2018). If Plaintiff fails to comply by December 21, 2018, this Court will deny him leave to proceed in forma pauperis and summarily dismiss this case. The Court instructs the Clerk to provide Plaintiff with a blank IFP application, along with a copy of this order. The Court defers ruling on Plaintiff's motion for attorney representation #4 . Plaintiff's " emergency motion to appoint a guardian and/or counsel for nonparty beneficiary" #5 is denied. Signed by the Honorable John Robert Blakey on 11/19/2018. (See Statement).
