ORDER: Plaintiff's application for leave to proceed in forma pauperis #3 is granted. The trust fund officer at Plaintiff's place of incarceration is authorized to deduct $16.00 from Plaintiff's account as the initial partial payment of the filing fee and to continue making deductions in accordance with this order. The Clerk shall send a copy of this order electronically to the Supervisor of Inmate Trust Fund Accounts at the Cook County Jail. For the reasons stated below, the complaint #1 is dismissed without prejudice for failure to state a federal claim. If Plaintiff wishes to proceed with this suit, he must by 12/17/2018 submit an amended complaint that states a viable claim against a proper defendant. If Plaintiff fails to comply, the dismissal will convert automatically to a dismissal with prejudice. The Clerk is directed to: (1) send Plaintiff an amended civil rights complaint form and a copy of this order; and (2) terminate Cook County Jail as a party defendant. Plaintiff must promptly submit a change-of address notification if he is transferred to another facility; failure to do so will lead to the dismissal with prejudice of this suit for want of prosecution.
