ORDER: Plaintiff's application for leave to proceed in forma pauperis #3 is denied without prejudice. Plaintiff is directed to either (1) submit a renewed application that is certified by a trust fund officer and that includes a copy of Plaintiff's trust fund statements showing all transactions in his account or accounts for the six-month period immediately preceding the date he responds to this order; or (2) pay the full statutory filing fee of $400.00. Failure to comply with this order by December 14, 2018, will result in summary dismissal of this case. Plaintiff also is advised that he must promptly submit a change-of-address notification if he is transferred to another facility or released. If Plaintiff fails to keep the Court informed of his address, this action will be subject to dismissal for failure to comply with a Court order and for failure to prosecute.
