ORDER:The U.S. Postal Service has returned mail sent to Plaintiff's address of record as undeliverable #7 . Although the Illinois Department of Corrections inmate locator indicates that Plaintiff was paroled on November 30, 2018, Plaintiff has not submitted a change-of-address notification. In light of Plaintiff's release from custody, by January 21, 2019, Plaintiff must submit a change-of-address notice with his current address. He must also either (1) submit a new application to proceed in forma pauperis with updated financial information or (2) pay the full $400 filing fee. If Plaintiff does not comply with all of these directives, this case will be summarily dismissed. The Clerk is directed to send Plaintiff a blank application to proceed in forma pauperis (for non-inmates), as well as copies of the December 7, 2018 order #6 and this order, to Plaintiff's address of record on the docket (that is, the Stateville Correctional Center). ORDER:Plaintiff's application for leave to proceed in forma pauperis #3 is denied without prejudice. Plaintiff must either: (1) submit a properly supported renewed in forma pauperis application that (a) is certified by trust fund officers at all facilities where he was incarcerated for the six months prior to submission of his complaint and (b) attaches copies of his trust fund statements showing activity in his accounts at all of those facilities for any portion of the six month period; or (2) pay the full statutory filing fee of $400. Failure to comply by January 9, 2019, will result in summary dismissal of this case. Plaintiff's motion for attorney representation #4 is denied without prejudice as it is too early to determine if recruitment of counsel is warranted.
