December 10, 2018 Opinion or Order Filing 6 ORDER: Plaintiff's application for leave to proceed in forma pauperis #3 is denied as incomplete and because it may be inaccurate in its current form. By January 4, 2019, Plaintiff must comply with the following. First, he must submit a written statement, under penalty of perjury, explaining:(A) why he should not be subject to sanctions, including dismissal and a referral to the court's Executive Committee for potential litigation restrictions, based on omissions and inaccurate information in his financial disclosures;(B) why he has not paid his previously incurred filing fee; and (C) why he should be allowed to initiate a new lawsuit in forma paupers in this court while his filing fee debt remains unpaid. Second, he must notify the court in writing whether he was incarcerated when he brought this lawsuit. Third, he must address the filing fee for this action by:(A) prepaying the $400 filing fee; or (B)if he continues to seek leave to proceed to proceed in forma pauperis, submitting an application that complies with the instructions in this order. Failure to comply by January 4, 2019 will result in dismissal. The court reminds Plaintiff as well, that he must notify the court of any address changes. Plaintiff's motion for attorney representation #4 is denied without prejudice to later renewal. If Plaintiff wishes to do so, he may make an in-person appointment with the U.S. District Court Hibbler Memorial Pro Se Assistance Program Self Help Attorney by selecting the "representing yourself" button on the Court's homepage(www.ilnd.uscourts.gov) and then following the instructions for making an appointment. Alternatively, Plaintiff may call (312)435-5691. The Clerk is directed to send Plaintiff an application for leave to proceed in forma pauperis along with this order. As it appears that Plaintiff is no longer incarcerated, the court expect him to appear in person or through obtained counsel at status dates or notice dates for motions.
