ORDER: Plaintiff's applications for leave to proceed in forma pauperis #3 #5 are granted. The trust fund officer at Plaintiff's current place of incarceration is authorized to deduct $9.21 from Plaintiff's account as the initial partial payment of the filing fee and to continue making deductions in accordance with this order. The Clerk shall send a copy of this order to the trust fund officer at the facility currently having custody of Plaintiff. Because Plaintiff has outstanding filing fees in multiple cases, the trust fund officer is reminded of the obligation to aggregate fee payments. The Court directs the Clerk to: (1) file Plaintiff's complaint #1 ; (2) issue summonses for service on Lieutenant Pieper and Officers Gonzalez, Campbell, Cassdenada, and Hunt; (3) send Plaintiff five blank USM-285 service forms, a magistrate judge consent form, filing instructions, and a copy ofthis order. Plaintiff must return the completed USM-285 service forms for all five Defendants to the Clerk in care of the Prisoner Correspondent by February 1, 2019. If he fails do so, the Court may dismiss any unserved Defendants, as well as dismiss this case for lack of prosecution. The Marshal is appointed to effect service and is authorized to send requests for waiver of service in the manner prescribed by Fed. R. Civ. P. 4(d) before attempting personal service. Plaintiff's motion for attorney representation #4 is denied without prejudice. Plaintiff is reminded that he must promptly submit a change-of address notification if he is transferred to another facility. Failure to do so may lead to dismissal of this action.
