December 21, 2018 Opinion or Order Filing 5 ORDER: Plaintiff's deficient application for leave to proceed in forma pauperis #3 is denied without prejudice. If Plaintiff wants to proceed with this action, he must either (1) prepay the $400 statutory filing fee; or (2) both show good cause in writing why he should be allowed to proceed in forma pauperis, and submit an application for leave to proceed in forma pauperis that complies with this order and shows the activity in his account from May 30, 2018, to the date he responds to this order. Plaintiff may pay by check or money order made payable to Clerk, United States District Court. Failure to comply with this order by February 1, 2018, will result in summary dismissal of this case. Plaintiff also is advised that he must promptly submit a change-of-address notification if he is transferred to another facility or released. If Plaintiff fails to keep the Court informed of his address, this action will be subject to dismissal for failure to comply with a Court order and for failure to prosecute. The Court defers consideration of Plaintiff's complaint and renewed motion for attorney representation #4 until his fee status has been resolved.
