By 3/18/2019, Plaintiff must either: (1) submit a completed in forma pauperis application that complies with the below instructions; or (2) pre-pay the full $400.00 statutory filing fee. If Plaintiff fails to comply with the foregoing instructions by that date, this case will be summarily dismissed.
