February 20, 2019	Filing 5 ORDER: Plaintiff's application for leave to proceed in forma pauperis #3 is granted. The trust fund officer at Plaintiff's place of incarceration is authorized to deduct $38.03 from Plaintiff's account as the initial partial payment of the filing fee and to continue making deductions in accordance with this order. The Clerk shall send a copy of this order electronically to the Supervisor of Inmate Trust Fund Accounts at the Cook County Jail. Summonses will not issue at this time as the Court dismisses Plaintiff's complaint #1 for failure to state a federal claim. By March 31st, 2019, Plaintiff may submit (1) an amended complaint (if he can do so consistent with Federal Rule of Civil Procedure 11) and (2) a completed USM-285 service form for each Defendant named in the amended complaint. If Plaintiff fails to comply, the Court will dismiss this case for failure to state a claim. The Clerk is directed to send Plaintiff an amended civil rights complaint form, a blank USM-285 form and instructions, and a copy of this order. Plaintiff must promptly submit a change-of address notification if he is transferred to another facility. Failure to do so may lead to dismissal of this action for want of prosecution.
