ORDER: Plaintiff's application for leave to proceed in forma pauperis #3 is granted. The Court orders the trust fund officer at Plaintiff's place of incarceration to deduct $8.66 from Plaintiff's account for payment to the Clerk of Court as an initial partial payment of the filing fee and to continue making monthly deductions in accordance with this order. The Court directs the Clerk of Court to: (1) send a copy of this order to the trust fund officer at the facility currently having custody of Plaintiff; and (2) amend the docket to reflect Plaintiff's last-reported address at Stateville's Northern Reception and Classification Center #5 . Summonses, however, shall not issue. Plaintiff's complaint #1 is dismissed without prejudice for failure to state a claim on which relief may be granted. If Plaintiff wants to proceed with this lawsuit, he must submit an amended complaint that states a federal claim against proper defendants. Failure to do so by 4-8-2019, will result in dismissal for failure to state a claim. Plaintiff also risks dismissal if he fails to keep the Court promptly apprised of any address change. The Court defers ruling on Plaintiff's motion for attorney representation. #4 The Clerk of Court is directed to send Plaintiff a civil rights amended complaint form along with a copy of this order.
