The application states that Plaintiff has received no income from any source over the past twelve months. In addition, the application states that Plaintiff pays for "gas, gym, and other incidental expenses." The Clerk is directed to send Plaintiff a blank in forma pauperis application form. If Plaintiff renews his in forma pauperis application, he must use the court's form. In addition, he must explain how he manages to pay for gas, gym, food, and shelter if, as he claims in his application, he does not have any income and has only $500 in his checking account. Plaintiff has until 2/26/2019 to file a renewed in forma pauperis application or to pay the $400.00 filing fee. Failure to comply will result in the summary dismissal of this case.
