Filing 5 ORDER: Plaintiff's application for leave to proceed in forma pauperis #3 is granted. The Court orders the trust fund officer at Plaintiff's place of incarceration to deduct $18.33 from Plaintiff's account for payment to the Clerk of Court as an initial partial filing fee, and to continue making monthly deductions in accordance with this order. The Court directs the Clerk to electronically send a copy of this order to the trust fund officer at the Cook County Jail. Summons however shall not issue. Plaintiff's complaint #1 is dismissed without prejudice for failing to state a claim upon which this Court can grant relief. If Plaintiff wants to proceed with this lawsuit, he must: (1) submit an amended complaint that both states a claim that can proceed and that names suable parties as Defendants and (2) submit a completed USM-285 (Marshals Service) form for each Defendant named in the amended complaint. An amended complaint is due by 4/19/2019. Failure to comply with these instructions will result in dismissal of this case for failure to state a claim. The Clerk is directed to send a copy of this order to Plaintiff, along with an amended civil rights complaint form and a blank USM-285 service form.
