April 8, 2019 Opinion or Order Filing 8 ORDER: Plaintiff DeAndre Collinss renewed application for leave to proceed in forma pauperis #6 is granted. The Court orders the trust fund officer at Collinss place of confinement to deduct $20.00 from Collinss account for payment to the Clerk as an initial partial payment of the filing fee, and to continue making monthly deductions in accordance with this order. The Clerk is directed to send a copy of this order electronically to the Supervisor of the Inmate Trust Fund Accounts at the Cook County Jail. Summonses, however, shall not issue. The Court dismisses Collinss complaint #1 without prejudice. Collins is granted until May 6, 2019, to submit an amended complaint and a completed USM-285 (Marshals service) form for each Defendant named in the amended complaint. If he fails to comply, the Court will dismiss the case for failure to state a claim. Collins is advised that he must promptly submit a change-of-address notification if he is released or transferred to another facility. If he does not keep the Clerk apprised of his correct address, this case may be dismissed. February 22, 2019 Opinion or Order Filing 5 ORDER: Plaintiff's applications for leave to proceed in forma pauperis ( #3 in Case Nos. 19 C 0881 and 19 C 0988) are denied without prejudice. To proceed with these cases, Collins must either (1) prepay the $400 filing fee for each case, for a total of $800; or (2) file fully and accurately completed in forma pauperis applications that attach certified trust fund ledgers from November 22, 2018, through the date he receives a certification and contain complete and accurate information for the entire twelve-month period before November 22, 2018. If he fails to comply by March 22, 2019, the Court will summarily dismiss these cases. The motions for attorney representation ( #4 in Case Nos. 19 C 0881 and 19 C 0988) are denied without prejudice to renewal should either case be allowed to proceed.
