February 28, 2019 Opinion or Order Filing 6 ORDER: Plaintiff's application for leave to proceed in forma pauperis #3 is granted. The Court orders the trust fund officer at Plaintiff's place of incarceration to deduct $4.33 from Plaintiff's account for payment to the Clerk of Court as an initial partial filing fee, and to continue making monthly deductions in accordance with this order. The Court directs the Clerk to send a copy of this order electronically to the Supervisor of the Inmate Trust Fund Accounts at the Cook County Jail, who is instructed to ensure that the Jail's records reflect all of the cases listed in this order and to make proper deductions. Summons however shall not issue. Plaintiff's complaint #1 is dismissed without prejudice for failing to state a federal claim. If Plaintiff wants to proceed with this lawsuit, he must: (l) submit an amended complaint that states a federal claim and (2) submit a completed USM-285 (Marshals Service) form for each Defendant named in the amended complaint. An amended complaint is due by 4-5-19. Failure to comply with these instructions will result in dismissal of this case-for failure to state a federal claim. The Clerk is directed to send a copy of this order to Plaintiff, along with an amended civil rights complaint form and a blank USM-285 service form.
