March 7, 2019 Filing 6 MINUTE entry before the Honorable Manish S. Shah: Plaintiff's motion for leave to proceed in forma pauperis #4 is denied, without prejudice. Plaintiff has filed an incomplete application. He failed to answer most of the questions on the form, and he reports no income since November 2017. Plaintiff must carefully and completely fill out a new application for leave to proceed in forma pauperis. Plaintiff must answer each and every question on the financial affidavit. Plaintiff should bear in mind that he is signing the document under penalty of perjury. In the alternative to filing a renewed i.f.p. application and financial affidavit, Plaintiff may choose instead to submit the statutory filing fee of $400.00. The Court directs Plaintiff either to (1) submit a renewed, properly completed i.f.p. application or (2) pay the statutory filing fee of $400.00. Failure to comply by April 4, 2019, will result in denial of leave to proceed in forma pauperis and summary dismissal of this case. The Court instructs the Clerk to provide Plaintiff with a blank i.f.p. application along with a copy of this order.
