ORDER signed by the Honorable Edmond E. Chang. For the reasons stated in the Order, Plaintiff's application to proceed in forma pauperis #3 is denied without prejudice because it is incomplete. Plaintiff is directed to either (1) submit a renewed complete application that is certified by a trust fund officer and includes a copy of his prison trust fund statement showing activity in his account for the six months immediately before the suit's filing (that is, from 08/27/2018 to 02/28/2019); or (2) pay the full statutory filing fee of $400.00. Failure to comply by 05/06/2019 will result in dismissal of this case. The Clerk of Court is directed to send Plaintiff a blank application to proceed in forma pauperis, along with a copy of this Order. The Clerk also shall send a copy of this order electronically to the trust fund officer at Plaintiff's current place of incarceration, who must ensure that deductions are being made for the two earlier cases in which Plaintiff was granted leave to proceed in forma pauperis but has not yet made any payments: (1) McCloud v. Keltner, No. 10 C 1374, (N.D. Ill.); and (2) McCloud v. Tomaszewski, et al., 10 C 4181 (N.D. Ill.), at a rate of ***40% of the monthly deposits*** in McCloud's account.
