ORDER :The Court grants Plaintiff's application to proceed in forma pauperis #3 and orders the trust fund officer at Plaintiff's place of incarceration to deduct $27.60 from Plaintiff's account for payment to the Clerk of Court as an initial partial filing fee, and to continue making monthly deductions in accordance with this order. The Court directs the Clerk to send a copy of this order electronically to the Supervisor of the Inmate Trust Fund Accounts at the Cook County Jail, who is instructed to ensure that the Jail's records reflect all of the cases listed in this order and to make proper deductions. The Clerk is also directed to: (1) file Plaintiff's complaint #1 ; (2) dismiss and terminate Defendants Cook County IL, Cermak Health Services, and Cook County Health Services; (3) add Defendant Sheriff Thomas Dart as a nominal Defendant to aid in identifying proper Defendants for this action; (4) correct the caption and title of the case to read, Pedro Martinez v. Sheriff Thomas Dart; (5) issue summons for service of the complaint on Defendant Dart; and (6) send Plaintiff a blank USM-285 (Marshals service) form, a magistrate judge consent form, filing instructions, and a copy of this order. Plaintiff must complete and return a USM-285 form for service on Defendant Dart. Failure to return the USM-285 form may result in dismissal of this case. Plaintiff also must promptly submit a change-of-address notification if he is transferred to another facility or released. If Plaintiff fails to keep the Court informed of his address, this action will be subject to dismissal for failure to comply with a Court order and for failure to prosecute. The Court appoints the U.S. Marshal to serve Defendant Dart. Plaintiff's motion for attorney representation #4 is denied without prejudice. Signed by the Honorable Matthew F. Kennelly on 5/1/2019.
