ORDER: The Court grants Plaintiff's application to proceed in forma pauperis #3 and orders the trust fund officer at Plaintiff's place of incarceration to deduct $26.00 from Plaintiff's account for payment to the Clerk of Court as an initial partial filing fee, and to continue making monthly deductions in accordance with this order. The Court directs the Clerk to send a copy of this order electronically to the Supervisor of the Inmate Trust Fund Accounts at the Cook County Jail, who is instructed to ensure that the Jail's records reflect all of the cases listed in this order and to make proper deductions. The Clerk of Court is also directed to: (1) file Plaintiff's complaint #1 ; (2) dismiss and terminate Defendants Cook County IL, Cook County Health Services, Cook County Board President Tony Preckwinkle and Sheriff Thomas Dart; (3) issue summonses for service of the complaint on Defendants Ramos and Davis using the USM-285 forms submitted by Plaintiff 5 ; and (4) send Plaintiff a magistrate judge consent form, filing instructions, and a copy of this order. Plaintiff must promptly submit a change-of-address notification if he is transferred to another facility or released. If Plaintiff fails to keep the Court informed of his address, this action will be subject to dismissal for failure to comply with a Court order and for failure to prosecute. The Court appoints the U.S. Marshal to serve Defendants using the USM-285 forms submitted by Plaintiff 5 . Plaintiff's motion for attorney representation #4 is denied without prejudice to renewal after Defendants respond to the complaint. Signed by the Honorable Matthew F. Kennelly on 5/2/2019.
