If Plaintiff wants to proceed with this lawsuit, he must submit a second amended complaint that complies with this order. ORDER: Plaintiff's application for leave to proceed in forma pauperis #3 is granted. The Court orders the trust fund officer at Plaintiff's place of incarceration to deduct $18.26 from Plaintiff's account for payment to the Clerk of Court as an initial partial payment of the filing fee and to continue making monthly deductions in accordance with this order. The Court directs the Clerk of Court to electronically send a copy of this order to the Supervisor of the Inmate Trust Fund Accounts at the Cook County Jail. Summonses shall not issue, however. Plaintiff's complaint #1 is dismissed without prejudice for failure to state a claim upon which relief may be granted. Plaintiff's failure to submit a proposed amended complaint by June 5, 2019, will result in dismissal of this matter with prejudice for failure to state a claim. The Clerk of Court is directed to send Plaintiff a blank amended complaint form and instructions along with a copy of this order.
