ORDER: Plaintiff's applications for leave to proceed in forma pauperis #3 #4 are denied as incomplete. To proceed with this case, by July 17, 2019, Plaintiff must either: (1) pay the $400 filing fee; or (2) submit both (a) an application for leave to proceed in forma pauperis that includes all information set forth below and (b) a written explanation as to why he cannot pay the filing fee by that date, despite his steady trust fund account deposits that, on average, exceed $200 per month, along with the information set forth below. Failure to comply will result in the summary dismissal of this case. Plaintiff is advised that he must promptly submit a change-of address notification if he is released or transferred to another facility.
