Filing 19 ORDER: For the reasons more fully explained in the order of the same date entered in Plaintiff's Case No. 19 C 2540, this case (No. 19 C 2614)which asserts the same claims against the same parties as Plaintiffs other caseis closed. See Serlin v. Arthur Andersen & Co., 3 F.3d 221, 223 (7th Cir. 1993) (a litigant cannot simultaneously proceed with duplicative suits; "a suit is duplicative if the 'claims, parties, and available relief do not significantly differ between the two actions'"). Filing 8 ORDER: Plaintiff's in forma pauperis (IFP) application #5 is denied as incomplete. The application includes neither Plaintiff's signature, nor a certificate from an authorized officer at his facility, nor a copy of his trust fund account statement "for the 6-month period immediately preceding the filing of the complaint." 28 U.S.C. 1915(a)(2). To proceed with this case, Plaintiff must either pay the $400 filing fee upfront or submit a completed and signed IFP application with a copy of his trust account statement. If he is granted IFP status, he will be assessed an initial partial payment followed by monthly deductions from his prison trust account. Proceeding IFP does not waive the filing fee, it simply allows a prisoner to pay it with monthly prison trust account deductions. This requirement applies for each filing fee Plaintiff owes. Plaintiff's failure to comply with this order by June 13, 2019 will result in summary dismissal of this case without prejudice. The Clerk shall send an IFP application form to Plaintiff at both Sheridan and Menard Correctional Centers. The Court will not conduct its initial review of Plaintiff's complaint until he has properly addressed the filing fee requirement. However, in addition to addressing the filing fee issue, Plaintiff should inform the Court if he intended to file two suits against Officers Finn and Hernandez. Although the complaint in this case and the amended complaint in Case No. 19 C 2540 are different, they name the same defendants and assert similar claims of retaliation and delays responding to Plaintiffs requests for crisis management.
