June 4, 2019 Opinion or Order Filing 12 ORDER: Plaintiff's application to proceed in forma pauperis #11 is granted. The Clerk shall file plaintiff's amended complaint #10 and issue summons. The court appoints the U.S. Marshal to serve defendants. The Marshal is authorized to send a request for waiver of service in the manner prescribed by Federal Rule of Civil Procedure 4(d) before attempting personal service. The Clerk shall mail one blank USM-285 form to plaintiff, and plaintiff must complete and return the forms by June 26, 2019. The U.S. Marshal will not serve defendants until the forms are returned, and the failure to return the forms by June 26, 2019, will lead to the dismissal of this case. April 29, 2019 Opinion or Order Filing 9 ORDER: Plaintiff's application to proceed in forma pauperis #4 is denied without prejudice because it is incomplete. Plaintiff must answer question 2.B. and provide a complete answer to questions 10 and 11 (describing the relationship to the people and the amount plaintiff contributes to their monthly support). In addition, plaintiff's complaint is dismissed without prejudice. Plaintiff must answer question 13 in the form complaint for employment discrimination, because the current complaint and the attached documents do not adequately describe what plaintiff's claim is about. Plaintiff should briefly explain what employment action was taken against her, when it happened, who was involved, and her basis for alleging that her race or sex was the reason the action was taken against her. The clerk shall send plaintiff one blank in forma pauperis application and one blank form complaint for employment discrimination. An amended complaint replaces the original complaint in its entirety, so plaintiff must answer every question completely and include all facts and documents she would like the court to consider in her amended complaint. Plaintiff's amended ifp application and amended complaint are due by May 28, 2019. If plaintiff does not comply by May 28, 2019, this case will be summarily dismissed.
