ORDER: Plaintiff's application for leave to proceed in forma pauperis #3 is granted. The Court orders the trust fund officer at Plaintiff's facility to deduct $23.50 from Plaintiff's account as an initial partial payment of the filing fee, and to continue making monthly deductions in accordance with this order. The Clerk of Court shall electronically send a copy of this order to the trust fund officer at the Cook County Jail. Plaintiff's complaint #1 , however, is dismissed without prejudice for failing to state a claim. See 28 U.S.C. 1915A. To proceed with this case, Plaintiff must submit an amended complaint both stating a claim of a constitutional violation and naming proper Defendants. An amended complaint is due by June 28, 2019. Plaintiff's failure to comply will result in summary dismissal of this case. The Clerk of Court shall send Plaintiff an amended complaint form.
