ORDER: The complaint #1 is dismissed without prejudice. Plaintiff Eric S. Smith's application for leave to proceed in forma pauperis #4 remains under review. The Clerk of Court is directed to: (1) amend the docket to reflect that Plaintiff Cornelius Duffeys name is Cornelious Duffie; (2) dismiss without prejudice the claims of all Plaintiffs except for the claims of Plaintiff Eric S. Smith, which will allow those Plaintiffs to bring their own separate lawsuits subject to the filing fee requirement set forth in the Prison Litigation Reform Act; (3) amend the docket to reflect that Eric S. Smith is the sole plaintiff; (4) send a copy of this order to all named Plaintiffs at their addresses of record at the Cook County Jail; (5) disable future notices to the dismissed Plaintiffs; and (6) send Plaintiff Eric S. Smith an amended civil rights complaint form. If Plaintiff Smith wishes to proceed with this case, he has until 7/1/2019 to: (1) either: (a) prepay the $400 filing fee by check or money order payable to the Clerk of Court; or (b) submit an under oath written statement providing all financial information and documents indicated below; (2) show cause in writing why this case should not be dismissed for a false allegation of poverty; and (3) submit an amended complaint on the Court's required form.
