July 11, 2019 Opinion or Order Filing 8 ORDER: Plaintiff's application for leave to proceed in forma pauperis #3 is granted. The Court orders the trust fund officer at Plaintiff's place of incarceration to deduct $10.83 from Plaintiff's account for payment to the Clerk of Court as an initial partial payment of the filing fee, and tocontinue making monthly deductions in accordance with this order. The Clerk of Court shall send a copy of this order to the trust fund officer at the Will County Adult Detention Facility. The Court notes that Plaintiff did not accurately complete the financial information in his application toproceed IFP. If he files suit in the future or files another IFP application in this case and submits inaccurate or fraudulent information, he may be subject to sanction including denial of IFP status or dismissal of the suit. The Court directs the Clerk of Court to (1) send (electronically if possible)a copy of this order to the trust fund officer at Plaintiff's current place of incarceration; (2) file Plaintiffs complaint #1 ; (3) terminate all defendants other than Dr. Kim from this action; and (4) send Plaintiff a copy of this order. Summons, however, shall not issue at this time. The plaintiff's motion for attorney representation #4 is granted. The Court requests that Bradley F. Rademaker, Neal, Gerber & Eisenberg, Two North LaSalle Street, Suite 1700, Chicago, IL, 60602, represent plaintiff in accordance with counsel's trial bar obligations under Local Rule 83.37. Counsel isgranted sixty days to advise the Court whether he will proceed on Plaintiff's complaint or file an amended complaint that comports with counsel's obligations under Rule 11 of the Federal Rules of Civil Procedure.
