Filing 4 ORDER: Plaintiff has submitted a complaint #1 but did not prepay the filing fee or seek leave to proceed in forma pauperis. By July 18, 2019, Plaintiff must either: (1) pay the full civil case filing fee of $400; or (2) submit a completed application for leave to proceed in forma pauperis that includes the documentation and certified trust fund account documents described below. If he does not comply, the Court will summarily dismiss this case. The Clerk is directed to send Plaintiff a blank application to proceed in forma pauperis, along with a copy of this order. Plaintiff must promptly submit a change-of address notification if he is released or transferred to another facility. If he does not keep the Clerk apprised of his correct address, this case will be subject to dismissal. Plaintiff's motion for attorney representation #3 is denied without prejudice to renewal after Plaintiff resolves the filing fee for this case. The Clerk is also directed to send a copy of this order to the trust fund officer at Plaintiff's present place of incarceration, who should ensure that collections resume as to Plaintiffs prior case, No. 14 C 6184, in which, as of May 31, 2019, Plaintiff had paid only $38.52 toward the $350 filing fee, leaving a balance due of $311.48; trust fund officers at any facilities where Plaintiff is confined now or in the future must ensure that fees for all of Plaintiff's cases are collected consistent with the Prison Litigation Reform Act and this Court's orders.
