ORDER :Plaintiff's application to proceed in forma pauperis (IFP) #6 is denied. As of 6/14/19, the date of Plaintiff's most recent IFP application, Plaintiff has at least $2,272.00. He is able to prepay the $400 filing fee and must do so to proceed with this case. He may pay the filing fee by check or money order made payable to the Clerk, U.S. District Court, Northern District of Illinois, 219 South Dearborn St., Chicago, Illinois 60604. Plaintiff must comply with this order by August 18, 2019, and his failure to do so will result in summary dismissal of this case without prejudice. See N.D. Ill. Local Rule 3.3(f). The Court will conduct its initial review of the complaint after the filing fee has been paid. Plaintiff's motion for attorney representation #4 is denied at this time, as no attorney is needed for Plaintiff to comply with this order. ORDER: Plaintiff's application to proceed in forma pauperis (IFP) #3 is denied without prejudice. The application is not signed, contains no certificate from an authorized officer at Plaintiff's facility attesting to the amount of funds he has in his account, and does not include a copy of Plaintiff's trust account statement. See Fed. R. Civ. P. 11(a) ("[e]very pleading... must be signed by at least one attorney of record... or by a party personally if the party is unrepresented); 28 U.S.C. 1915(a)(2) (a prisoner's IFP application must include "a certified copy of the trust fund account statement (or institutional equivalent) for the prisoner for the 6-month period immediately preceding the filing of the complaint"). The Prison Litigation Reform Act requires prisoners to pay the filing fee for each case he brings, either upfront or with monthly deductions from his facility's trust account. Plaintiff must provide the above listed information with his IFP application so the Court can determine if qualifies for pauper status and how much his initial payment will be. By June 30, 2019, Plaintiff must either prepay the filing fee ($400) or file a completed, signed IFP application with both a statement of his trust account and a certificate form an authorized officer to pay the fee ($350 for IFP litigants) in monthly installments. Plaintiff's failure to comply will result in summary dismissal of this case without prejudice. The Clerk shall send an IFP application form to Plaintiff.
