ORDER :Plaintiff's application for leave to proceed in forma pauperis #3 is granted. The Court orders the trust fund officer at Plaintiff's place of incarceration to immediately deduct $3.17 from Plaintiff's account for payment to the Clerk of Court as an initial partial payment of the filing fee and to continue making monthly deductions in accordance with this order. The Court directs the Clerk of Court to send a copy of this order to the supervisor of the inmate trust account office at the Kane County Jail and to the Court's Fiscal Department. Summons, however, shall not issue. Plaintiff's complaint #1 is dismissed without prejudice for failure to state a claim. If Plaintiff seeks to continue with this case, he must, by August 16, 2019, submit: (1) an amended complaint that rectifies the deficiencies explained below; and (2) a separate completed USM-285 (Marshals Service) form for each Defendant named in the amended complaint. Plaintiff's failure to comply with this order by that date will result in dismissal of this case. The Clerk of Court is directed to send Plaintiff a civil rights amended complaint form and one USM-285 (Marshals Service) form, of which Plaintiff may make copies if needed, along with a copy of this order. Ruling on Plaintiff's motion for attorney representation #4 is deferred. Plaintiff's motions for discovery requests #5 and #6 are denied as premature. The Clerk of Court is directed to send Plaintiff an amended complaint form, a blank USM-285 form, and instructions along with a copy of this order. Signed by the Honorable John J.
