Filing 6 AMENDED ORDER signed by the Honorable Sharon Johnson Coleman on 7/29/2019: Plaintiff's application for leave to proceed in forma pauperis #3 is denied as incomplete. If Plaintiff wishes to pursue this litigation, he must address the filing fee for this action, either by (1) both submitting a completed in forma pauperis application that is certified by an appropriate jail official and includes a trust fund account activity statement covering December 14, 2018, through the date of his renewed application and a written IFP Statement explaining why he should be permitted to proceed without prepayment of the filing fee when he received more than $31,000 in income in the preceding year; or (2) pre-paying the full $400.00 statutory filing fee. Even if he pays the fee, however, this case will not necessarily proceed as Plaintiff must also submit a written Litigation History Statement explaining why this case should not be dismissed with prejudice for his failure to fully disclose his litigation history. Failure to further address the filing fee and submit a Litigation History Statement by August 30, 2019, will result in dismissal of this case. The Clerk is directed to send a copy of this order to Plaintiff, along with an in forma pauperis application form.
