July 17, 2019 Opinion or Order Filing 5 ORDER: Plaintiff's application for leave to proceed in forma pauperis #3 is granted. The initial filing fee is waived. The trust fund officer at Plaintiff's place of incarceration is authorized to make deductions from Plaintiff's account in accordance with this order. The Clerk shall send a copy of this order electronically to the Supervisor of Inmate Trust Fund Accounts at the Cook County Jail and to the Court's fiscal department. Summonses will not issue at this time as the Court dismisses Plaintiff's complaint #1 for failure to state a federal claim. By Augsut 19, 2019, Plaintiff may submit (1) an amended complaint and (2) a completed USM-285 service form for each defendant named in the amended complaint. If Plaintiff fails to comply, the Court will dismiss this case for failure to state a claim. The Clerk is directed to send Plaintiff an amended civil rights complaint form, a blank USM-285 form and instructions, and a copy of this order. Plaintiff must promptly submit a change-of address notification if he is transferred to another facility. Failure to do so may lead to dismissal of this action for want of prosecution.
