Filing 5 ORDER: Plaintiffs application for leave to proceed in forma pauperis #3 is granted. The trust fund officer at the Cook County Jail is authorized to deduct $3.80 from Plaintiffs account as the initial partial payment of the filing fee and to continue making deductions in accordance with this order as set forth below. The Court directs the Clerk to: (1) send a copy of this order electronically to the Supervisor of Inmate Trust Fund Accounts at the Cook County Jail; (2) file Plaintiffs complaint #1 ; (3) issue summonses for service of the complaint on Defendants; and (4) send Plaintiff two blank USM-285 service forms, a magistrate judge consent form, filing instructions, and a copy of this order. Plaintiff must return the completed USM-285 service forms to the Clerk in care of the Prisoner Correspondent. The Marshal is appointed to effect service and is authorized to send a request for waiver of service in the manner prescribed by Fed. R. Civ. P. 4(d) before attempting personal service.
