ORDER: The Court denies Petitioner's motion for leave to file in forma pauperis #3 without prejudice to reconsideration should he renew his motion in compliance with this order. Petitioner is given until August 22, 2019 either to file an in forma pauperis application on the enclosed form with the information required by 28 U.S.C 1915(a)(2) or to pay the $5.00 filing fee. If Petitioner takes no action by that date, the Court may dismiss this action without prejudice. The Clerk is instructed to: (1) terminate Respondent Senator Kwame Raoul Attorney General of the State of Illinois from the docket; (2) alter the docket to reflect that Respondent is Frank Lawrence, Acting Warden, Menard Correctional Center; and, (3) send Petitioner a blank in forma pauperis application with instructions.
