ORDER: Plaintiff's application for leave to proceed in forma pauperis #3 is granted. Plaintiff's additional application for leave to proceed in forma pauperis #5 is stricken as moot. The Court orders the trust fund officer at Plaintiff's place of incarceration to deduct $8.33 from Plaintiff's account for payment to the Clerk of Court as an initial partial payment of the filing fee and to continue making monthly deductions in accordance with this order. The Clerk of Court is directed to electronically send a copy of this order to the Supervisor of the Inmate Trust Fund Accounts at the Cook County Jail, Summonses, however, shall not issue. Plaintiff's amended complaint #4 is dismissed with prejudice for failure to state a claim on which relief may be granted. The dismissal counts as one of Plaintiff's three allotted dismissals under 28 U.S.C. 1915(g). Final judgment shall enter. Civil case terminated. Signed by the Honorable Robert W.
