ORDER: Plaintiff's Application for Leave to Proceed In Forma Pauperis #4 and Motion for Attorney Representation [ 5] are denied. In her in forma pauperis application, Plaintiff has failed to list cases that she has previously filed in this district. Having reviewed docket records, the Court has identified several cases that Plaintiff has previously filed in this district, for example, ll-cv- 05653, 14-cv-03671, 09-cv-08060, 04-cv-02618. The Court must require honesty and forthrightness from persons proceeding in forma pauperis. See Chung v. Dushane, No. 03 C 5955, 2003 WL 22902561, at *2 (N.D. Ill. Dec. 9, 2003), aff'd sub nom. Chung v. KPMG LLP, 104 F. App'x 576 (7th Cir. 2004) (citing Denton v. Hernandez, 504 U.S. 25, 27 (1992)). "Plaintiffs who attempt to deceive federal... judges cannot expect favorable treatment in matters of discretion. Campbell v. Clarke, 481 F.3d 967, 969 (7th Cir. 2007). Accordingly, because Plaintiff failed to complete her application by not listing the multiple cases that she has filed, her motion to proceed in forma pauperis is denied.
