ORDER: Plaintiff' motion to proceed in forma pauperis #3 is granted. The trust fund officer at his place of confinement is directed to deduct $26.00 as an initial partial payment of the filing fee and to continue deducting from Plaintiff's trust account in accordance with this order. The Clerk shall: (1) send this order to the trust fund officer at Lawrence Correctional Center; (2) file Plaintiff's complaint #1 ; (3) issue summonses for its service on Stateville Officers Maurice Boler and A. Thomas; (4) dismiss all other Defendants; and (5) send to Plaintiff two USM 285 service forms, a Magistrate Judge consent form, and filing instructions. The U.S. Marshal is appointed to serve Defendants, but completed service forms are needed before service is attempted. Plaintiff must return completed service forms to the Court by October 28, 2019; his failure to comply may result in dismissal of this case.
