Filing 9 ORDER: Plaintiff's application for leave to proceed in forma pauperis #4 is denied without prejudice. Plaintiff is ordered to submit a properly completed in forma pauperis application with the supporting documentation required by 28 U.S.C. 1915(a)(2) or pay the full $400 filing fee.
