Filing 5 ORDER: Plaintiff's application for leave to proceed in forma pauperis #3 is granted. The Court orders the trust fund officer at Plaintiff's place of incarceration to deduct $3.73 from Plaintiff's account for payment to the Clerk of Court as an initial partial payment of the filing fee and to continue making monthly deductions in accordance with this order. The Court directs the Clerk to send a copy of this order (electronically if possible) to the trust fund officer at the facility having custody of Plaintiff, as well as the Court's fiscal department. Summonses, however, shall not issue at this time as the Court dismisses Plaintiff's complaint #1 . Plaintiff is granted until December 6, 2019, to submit an amended complaint raising a properly joined claim or set of claims plus a completed USM-285 (Marshals service) form for each Defendant named in the amended complaint. If he does not do so, the Court will summarily dismiss this case. Plaintiff's motion for attorney representation #4 , which reflects that he has made no attempt to obtain counsel on his own, is denied without prejudice. See Pickett v. Chicago Transit Auth., 930 F.3d 869, 871 (7th Cir. 2019) (citing Pruitt v. Mote, 503 F.3d 647, 654 (7th Cir. 2007) (en banc)). Plaintiff must promptly submit a change-of-address notification if he is released or transferred to another facility. If he fails to comply, this case will be subject to dismissal with no further warning. The Clerk is directed to send Plaintiff an amended civil rights complaint form and instructions, a blank USM-285 form, and a copy of this order. Signed by the Honorable Elaine E.
