ORDER: Plaintiff's application for leave to proceed in forma pauperis #3 is denied without prejudice. Plaintiff must either: (1) pay the full statutory filing fee of $400; or (2) submit a fully and accurately completed renewed in forma pauperis application that attached certified trust fund ledgers beginning July 4, 2019 (when he was booked into the Cook County Jail) and ending on the date that he complies with this order. If Plaintiff fails to comply by January 6, 2020, the Court will summarily dismiss this lawsuit Plaintiff's motion for attorney representation #4 is denied without prejudice. Finally, consistent with Prison Litigation Reform Act and this Court's orders, the trust fund officer at any facility where Plaintiff is confined must ensure the proper deductions are made, for all Plaintiff's cases, including Jones v. Galiardo, Case No. 16 C 3891 (N.D. ILL.) (Alonso, J.). As detailed in this order, Plaintiff owes $350 in Case No. 16 c 3891 as of November 26, 2019. The Clerk is directed to send a blank application for leave to proceed in forma pauperis and a copy of this order to Plaintiff, and a copy of this order to the trust fund officer at the Cook County Jail, who should correct the Jail's records and ensure that proper deductions are made.
