Filing 6 ORDER: Plaintiff's application for leave to proceed in forma pauperis #3 is granted. The trust fund officer at Plaintiff's place of custody shall deduct $20.88 from Plaintiff's account for payment to the Clerk as an initial partial payment of the filing fee, and continue making monthly deductions in accordance with this order. The Clerk shall send a copy of this order (electronically if possible) to the trust fund officer at the Lake County Jail and the Court's fiscal department. The Clerk shall: (1) file Plaintiff's complaint #1 ; (2) issue summonses for service of the complaint #1 on Nurse Esther, Armor Correctional Healthcare, and Lake County, and correct the docket to reflect that they are the only active Defendants; (3) forward the USM-285 service form for Lake County 5 to the Marshals Service; and (4) send Plaintiff two blank USM-285 service forms, a magistrate judge consent form, filing instructions, and a copy of this order. Plaintiff must return completed USM-285 service forms for Nurse Esther and Armor to the Clerk in care of the Prisoner Correspondent by January 16, 2020. When completing the form for Nurse Esther, Plaintiff should provide as much identifying information as he can (for example, a last name, physical description, information about the shifts she normally works, etc.). If he fails to return the service forms, the Court may dismiss Nurse Esther and Armor, as well as dismiss this case for lack of prosecution. The Marshal is appointed to serve defendants and is authorized to send requests for waiver of service in the manner prescribed by Fed. R. Civ. P. 4(d) before attempting personal service. Plaintiff's motion for attorney representation #4 is denied without prejudice as he appears to be able to complete the next steps in this case at this time. The court will revisit recruitment of counsel after Defendants respond to the complaint. Plaintiff must promptly submit a change-of-address notification if he is released or transferred to another facility.
