ORDER: The district court for the Central District of Illinois has transferred this case to this court. The Clerk is instructed to change the nature of suit from "550 Prisoner: Civil Rights" to "555 Civil Rights (Prison Condition)." The Central District of Illinois has granted Plaintiff's motion for leave to proceed in forma pauperis #8 . Future payments of the filing fee are to be directed to this court. The Clerk shall forward a copy of this order to the trust fund officer at the Southwestern Illinois Correctional Center in East St. Louis, Illinois, and to the court's Fiscal Department. Summons, however, shall not issue at this time as Plaintiff's complaint #1 is dismissed without prejudice. Plaintiff is granted until February 17, 2020, to submit an amended complaint and a completed USM-285 (Marshals service) form for each Defendant named in any amended complaint. If he fails to comply, this case may be dismissed. Plaintiff is advised that he must promptly submit a change-of-address notification if he is released or transferred to another facility. If he does not keep the Clerk apprised of his correct address, this case may be dismissed. The Clerk is directed to send Plaintiff an amended civil rights complaint form and instructions, a blank USM-285 form, and a copy of this order. TEXT ORDER granting #8 Petition to Proceed In Forma Pauperis. Pursuant to 28 U.S.C. Section 1915(b)(1), Plaintiff is assessed an initial partial filing fee of $21.15. The agency having custody of Plaintiff is directed to forward the initial partial filing fee from Plaintiff's account to the Clerk of Court. After payment of the initial partial filing fee (or immediately if no funds are available for that payment) the agency having custody of Plaintiff is directed to make monthly payments of 20 percent of the preceding month's income credited to Plaintiff's account to the Clerk of Court. Income includes all deposits from any source. The agency having custody of the plaintiff shall forward these payments each time Plaintiff's account exceeds $10, until the filing fee of $350 is paid in full. Plaintiff will have 30 days in which to pay the $400 filing fee or submit a corrected petition identifying the sources of his income.
