Filing 7 ORDER: Plaintiff John Pipkin filed this pro se civil rights action about his medical treatment at Cook County Jail. By order dated January 23, 2020 #5 , the Court denied Plaintiff's application to proceed in forma pauperis (IFP) and instructed Plaintiff to either refile a complete IFP application or pay the filing fee. The Clerk sent a copy of the January 23, 2020 order to Plaintiff at Cook County Jail via the United States Postal Service, which has returned it with the notation that it is undeliverable and that no forwarding address is available #6 . Based on searches of the Cook County Jail and IDOC inmate locators, it appears Plaintiff may no longer be incarcerated. Plaintiff has since initiated a second lawsuit, Pipkin v. Dart, No. 1:20-cv-0920 (N.D. Ill.) (Tharp, J.), in which he provides an alternate address. Plaintiff has an obligation to advise the Court of any changes to his address. By March 13, 2020, Plaintiff must provide the Court with an updated mailing address. If he does not do so, this case will be summarily dismissed on the basis that Plaintiff has abandoned his interest in pursuing it. The Court will extend the deadline for Plaintiff to submit a renewed IFP application or pay the filing fee to March 13, 2020. The Clerk is directed to forward a copy of this order and the January 23, 2020 order (Dkt. 5) to Plaintiff's Cook County Jail address and to the address provided in case no. 1:20-cv-0920: 2940 S. State St, Apt. 606, Chicago IL 60616. Signed by the Honorable John J. Tharp, Jr on 2/12/2020. Mailed notice (cn). |
MAILED copy of Orders #5 and #7 to Plaintiff both at the Cook County Jail and at 2940 S. State St. Apt 606, Chicago, IL 60616 (cn). |
Filing 6 MAIL RETURNED, for document #5 sent to John Pipkin returned as undeliverable, return to sender. No new contact information received. (sxb, ) |
Filing 5 ORDER: Plaintiff's application for leave to proceed in forma pauperis #3 is denied without prejudice. If Plaintiff wants to proceed with this lawsuit, he must either (1) submit a renewed application that is signed by him, accompanied by a copy of his trust fund statements showing activity in his account or accounts for the entire six-month period immediately preceding his initiation of this action, and certified by an appropriate official; or (2) pay the full statutory filing fee of $400.00. Failure to comply with this order by February 28, 2020, will result in summary dismissal of this case. Plaintiff also must promptly submit a change-of-address notification if he is transferred to another facility or released. Failure to keep the Court informed of his address may lead to dismissal of this action for failure to comply with a Court order and for want of prosecution. The Clerk of Court is directed to send Plaintiff a blank application to proceed in forma pauperis and a copy of this order. Signed by the Honorable John J. Tharp, Jr on 1/23/2020. Mailed notice.
