Filing 9 MINUTE entry before the Honorable Edmond E. Chang: Plaintiff's motion #4 to proceed in forma pauperis is denied without prejudice because parts of the financial affidavit are incomplete. First, in Question 1, the form requires plaintiffs who are not currently employed to provide information about their last employment; Plaintiff left that information blank. R. 4 at 1. Second, in Question 4, Plaintiff checked the "Yes" line for cash in a bank account, but then did not say how much. R. 4 at 2. Third, Plaintiff listed what appears to be her residence in the real-estate section (Question 5 on other assets), but does not list its approximate value. Lastly, there is a reference to Social Security in the answer to Question 8, but in Question 3 the line for public assistance is blank; if Plaintiff is receiving Social Security benefits, they should be listed there. The Clerk's Office shall mail a blank in forma pauperis application to Plaintiff. If Plaintiff wishes to renew the application, then she must file the application by 04/06/2020; otherwise, she must pay the $400 filing fee by that date. Failure to file the application or pay the fee will result in dismissal of the case. Separately, Plaintiff's motion #5 for attorney representation is denied without prejudice in light of (1) the incomplete financial affidavit; (2) the motion does not list any lawyers or legal services organizations contacted by Plaintiff (she should try asking at least three lawyers or organizations for pro bono (that is, free) or contingency-fee representation; and (3) at this early stage of the case, Plaintiff can represent herself. She has some college application, R. 5 at 2, and the administrative Charge of Discrimination sets forth the basic facts of the claim (the Charge was probably drafted by an intake employee, but it shows that Plaintiff can articulate the facts at this early stage of the case).
