Filing 6 ORDER: On March 26, 2020, Plaintiff filed a motion (Dkt. No. 1) on the Court's docket reserved for emergency motions during the COVID-19 public health crisis. (See Dkt. No. 3.) The motion was then docketed as a new civil filing in the present case. It is unclear from Plaintiff's submission if he intended to initiate a new lawsuit or if he meant to file the submission as a motion in his other open case, Jobes v. Dart, No. 1:20-cv-0812 (N.D. Ill.) (Wood, J.). By 6/29/2020, Plaintiff must inform the Court in writing whether he wishes to continue with the present lawsuit. If Plaintiff intended to begin a new lawsuit, by 6/29/2020, he must submit his claims, including his litigation history, on the Court's complaint form and either: (1) submit a completed application for leave to proceed in forma pauperis that includes a certified copy of the trust fund account statement (or institutional equivalent) for the six-month period immediately preceding the filing of the complaint and a completed certification from the appropriate official of each prison where Plaintiff is or was confined during the last six months; or (2) pay the full statutory filing fee of $400. If he does not comply, the Court will summarily dismiss this case. The Clerk of Court is directed to send Plaintiff a blank civil rights complaint form, instructions, a blank application to proceed in forma pauperis, and a copy of this Order.
